To ensure we have your most updated and accurate information, we request that you complete and return the attached New Vendor Form, signed W-9 and any Applicable Certifications.

All vendors must comply with the Illinois Gaming Board rules/regulations and obtain proper licensing, prior to conducting business with Golden Nugget Danville.
